Change of Name

An adult, over the age of 18, may change his/her last name (family name) and/or first name (private name).
The change is subject to the approval of the Ministry of Interiors.
One who changes his name may not, by law, change it again for 7 years.


Requirements for change of name:

1. You must submit the request, IN PERSON, at the Consulate.

2. One complete Notice form for a change/choice of name.
3.Processing fees: Change of first or family name $26 Addisional family name $26
4. Please note that the change is subject to the approval of the Ministry of Interiors and your old name will remain UNTIL confirmation. Once you receive the confirmation please contact the Consular Department.
